Understanding Our Terms: Consent Agreement

By proceeding with lessons, you acknowledge your understanding and acceptance of the following terms. A separate, signable copy of this agreement will be provided.

1. Teaching Environment

   I understand and agree that classes will be conducted at Travis Osterloh's home. I acknowledge that this is a private residence and I am comfortable with this arrangement.

2. Attendance Policy

   I agree to adhere to the sign-up and attendance policy as detailed below. I understand that consistent attendance is necessary for my progress and success in the program.

3. Use of Photographs and Achievements

   I grant Travis Osterloh permission to use any photographs or recordings of my participation, as well as my achievements, for marketing purposes. This includes, but is not limited to, promotional materials, social media, and website content. I understand I can opt-out of this by specific written request.

4. Presence of a Dog

   I acknowledge that there is a dog in the household, and the dog may be present in or pass through the teaching area. I understand that the dog is part of the household environment and agree to this arrangement.

5. Liability Waiver

   I understand that I will be asked to sign a liability waiver releasing and discharging Travis Osterloh, his family, and any other associated persons from any and all liability, claims, demands, or causes of action that may arise from any injury, loss, or damage sustained by me while on the premises. This includes, but is not limited to, injuries sustained while walking in and out of the teaching area and any incidents involving the household dog.

6. Emergency Contact Information

   I understand I will be required to provide emergency contact information (name, relationship, phone, email) on a separate form.

7. Health and Safety Acknowledgment

   I agree to follow all health and safety guidelines as provided by Travis Osterloh. It is my responsibility to inform the instructor of any medical conditions or allergies that may require special attention.

8. Code of Conduct

   I agree to adhere to the expected behavior and conduct during classes. This includes respecting others, being punctual, and following any specific house rules communicated by Travis Osterloh.

9. Confidentiality

   I agree to maintain the confidentiality of any sensitive or proprietary information shared during the sessions.

10. Medical Emergencies

    In the event of a medical emergency, I authorize Travis Osterloh to administer basic first aid and contact emergency services if necessary.

11. Acknowledgment of COVID-19 Safety Protocols

    I acknowledge and agree to adhere to any COVID-19 safety protocols, such as mask-wearing, hand sanitizing, or social distancing, as communicated by Travis Osterloh, should they be in effect.

12. Parental/Guardian Consent for Minors

    If the student is a minor, the parent or guardian must provide their contact information and acknowledge these terms on the separate, signable consent form.

13. Membership and Payment Terms

    I understand and agree to the following terms regarding membership and payments:

    1. Subscription Basis: Memberships are charged on a month-to-month subscription basis.
    2. Automatic Payments: I authorize Travis Osterloh to automatically charge the provided payment method for the ongoing monthly subscription.
    3. Class Usage: All scheduled classes within a paid monthly period must be used before the next billing cycle. Unused classes from a previous billing cycle do not typically roll over unless a specific exception is granted by Travis Osterloh.
    4. Billing Cycle: Payments will be withdrawn automatically each month on or around the same date as the initial sign-up.
    5. Cancellation Policy: To cancel membership and stop all future payments, I must personally contact Travis Osterloh to request cancellation. A notice period (e.g., by the 15th of the month to cancel for the following month) may apply and will be confirmed upon cancellation request.
    6. Refunds: Due to the nature of the services, all payments for subscription periods are non-refundable.

Sign-Up and Attendance Policy

1. Enrollment:
   1. Students can enroll by completing the registration process, which includes filling out a registration form, agreeing to the consent terms, and submitting payment for the first subscription period.
   2. Enrollment is subject to availability, as class sizes are limited.
2. Attendance:
   1. Regular attendance is vital for progress. We expect all students to attend their scheduled classes punctually and consistently.
   2. In case of absence, students are encouraged to notify Travis Osterloh in advance, if possible, or as soon as possible after the absence. This allows for effective planning.
   3. If a student consistently misses classes without notification or a valid reason, Travis Osterloh reserves the right to discuss the student's continued enrollment.
3. Make-Up Classes:
   1. We understand that unforeseen circumstances may arise. As a courtesy, make-up classes for missed lessons may be offered, subject to availability and Travis Osterloh's discretion.
   2. Make-up classes are scheduled at mutually convenient times and are contingent upon instructor availability. Make-up classes cannot be guaranteed for all missed lessons.
4. Cancellations and Rescheduling (by Student):
   1. If a student needs to cancel or reschedule a class, we request a minimum notice period of 24 hours.
   2. Short-notice cancellations (less than 24 hours) or no-shows will typically result in forfeiture of that lesson, as the time slot was reserved. Frequent rescheduling may impact the availability of preferred slots.
5. Refunds and Withdrawals:

   As stated in the Membership and Payment Terms, subscription payments are non-refundable. If you choose to withdraw, please follow the cancellation policy by contacting Travis Osterloh personally.

6. Code of Conduct:
   1. Students are expected to adhere to a code of conduct that promotes a positive and respectful learning environment.
   2. Disruptive behavior, harassment, or any actions that compromise the safety and well-being of others will not be tolerated and may result in disciplinary action, including potential dismissal from the program without refund.
7. Communication:

   Effective communication is essential. Students and parents/guardians are encouraged to maintain open lines of communication with Travis Osterloh regarding any concerns, questions, or feedback.

By enrolling in Osterloh Guitar Academy, you acknowledge that you have read, understood, and agree to abide by the above sign-up and attendance policy. This policy ensures a fair and conducive learning environment for all students and helps maintain the quality of our programs.
